| What you can expect on your first visit: |
We are very pleased that you have chosen our offices to serve your health care needs. During your first visit to our office you will be asked to fill out a patient information form. For your convenience this form is available on our website, if you would like to complete it before your arrival. It is a comprehensive form that asks for personal information, insurance information, health history and current health issues. If you have any questions about the form our staff is always available to help. Please be sure to have the following items with you, your insurance card and if available a driver’s license or picture ID.
After the Doctor has read through the information that you provided he will sit down with you and discuss your case. He may need more detail on certain things to help determine the cause of your condition. During this initial consultation the Doctor will answer any additional questions you may have, then you will have a chiropractic examination. This may include a variety of tests such as your reflexes, ability to turn and bend, as well as other orthopedic, neurological, postural, and physical examinations. Blood pressure screening is also included as a part of your examination. If necessary, the Doctor will write you a script for X-rays or an MRI.
After your examine he will discuss with you how chiropractic care can be the most beneficial to your and your health. If the doctor determines that chiropractic care is not the right choice for you at this time, he will refer you to another medical professional to seek further treatment. If the Doctor feels chiropractic care is appropriate for you, you will be treated on your first visit.
During your initial visit we will make every effort to answer any all of your questions or concerns. Please remember that we are always available to our patients to answer any questions that may come up after you have left the office.
